APPLICATION FORM PDF —all 5 pages need to be filled out and returned as soon as possible (by mail or email) to Cindy Johnson, our Epiphany Facility Liason. She will make sure that the Facility receives these and they will then begin the process for scheduling fingerprinting. (The entire approval process can sometimes take a couple of months, so it's important to start immediately.)

Anyone who is interested in serving on the team needs to also email or call Lay Director Linda Roberts and let her know immediately. We are AGGRESSIVELY RECRUITING team members, so if someone has a desire to serve I'm sure that we can find a place for them to minister. Serving on an Epiphany Team (whether on the inside or the outside) is such a blessing!
